All co-payments, deductibles, and patient responsibility amounts are due at the time of check-in. We accept cash, check, or card payments. (If a check is returned for any reason, you should know that there will be a $10 fee assessed.)
Payments can be made by contacting our billing and insurance department at (515)241-2010. You can also submit payments through the patient portal at URL: dermatologypc.ema.md or through the link on our website www.dermatologypc.com.
We request you present your current insurance cards at the time of Check-In. Our staff will make copies of all your active medical insurance cards (primary, secondary, and tertiary) to confirm accurate insurance carrier information and coverage.
It is your responsibility to notify our office promptly of any patient changes (ie, address, name, insurance information) to facilitate appropriate billing for the services rendered to you. Failure to provide complete and accurate insurance information may result in the entire bill being categorized as the patient’s responsibility.
If you are unsure of your financial responsibility, please contact your insurance company in advance to obtain this information. Any balance remaining after insurance has paid its covered portion will be due upon receipt of a statement.

Dermatology, P.C. accepts most major insurance plans. However, with the frequent changes that happen in the insurance marketplace, it is a good idea for you to contact your insurance company before your appointment to verify Dermatology, P.C. is a participating provider of your plan.

Dermatology, P.C. accepts “traditional” Medicare assignment. You are responsible for the applicable coinsurance, deductibles, and charges for non-covered services. In addition to the bill we send, you should also receive an explanation of benefits from Medicare indicating how much you owe.
Medicare Advantage plans. Dermatology, P.C. participates with most, but not all, Medicare Advantage plans. Please check to see if Dermatology, P.C. is a participating provider and if you require a formal referral through your primary care provider prior to arriving or scheduling an appointment. *Bring your insurance cards.
Dermatology, P.C. accepts assignment for most major insurances. You will be required to pay applicable co-pay amounts at the time of service. You are responsible for any co-insurance, deductibles, and for all unpaid noncovered services.
If Dermatology, P.C. participates with your Medicaid assignment, a current Medicaid card must be presented at each visit, and if applicable, you will be required to pay the co-pay at the time of service. If you have exceeded the legislative limits for the year per Medicaid, you will be held responsible for the charges. If you have a Medicaid HMO, please obtain your referral from your primary care provider at least two weeks before your appointment.
If Dermatology, P.C. participates with your insurance provider, you will be required to pay the applicable co-pay at the time of your appointment. If your insurance plan requires a referral from your primary care provider (PCP) or a pre-authorization from your insurance, you will need to contact your PCP to be sure it has been obtained. If we do not receive the referral or pre-authorization 2 weeks before your scheduled appointment, your appointment will be subject to cancellation or rescheduling.
We are pleased to provide services to patients who do not have insurance. However, you will be expected to pay for all services provided to you in full at the time of service. Please contact our billing and insurance department at (515)241-2010 before scheduling to discuss Good Faith Estimates (GFE) for an office visit. If additional services are required after the office visit, a GFE for additional services will be drafted and reviewed before receiving additional treatment.
Dermatology, P.C. is supported by a staff of dedicated professionals who can assist you with most financial questions. Our goal is to help relieve the patient/caregiver of burdensome paperwork. We are happy to assist you in understanding questionsthat you may have about fees, our policies, or your responsibilities.

When you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance.
